A step splits into several objectives
One step can list several objectives, and every one of them counts.
One route per run
Into the Shadows activates one escape route per run, so the other two stay inactive.
An objective can fail the run
Some objectives fail instead of completing. Each chapter walkthrough lists its own failure conditions.
A Challenge did not count
Story Challenges are extra goals. They never block Story progress.
The step needs a place or a character
Some objectives start when you reach a named area, or when the character they name is involved.
